Prosthetics advances made for war hold hope for Boston victims

Those who lost their limbs in the Boston Marathon bombing are finding support among military veterans who have gone through similar things, NBC's Lester Holt reports.

Boston Marathon spectators who lost limbs in the bombings stand to benefit from years of advances in prosthetic medicine made at Walter Reed Military Medical Center in Bethesda, Md.

Calvin Todd, 26, is among those who have discovered a new life with the help of doctors at Walter Reed. The army medic was on foot patrol in Afghanistan in October 2012 when he stepped on an explosive.

?I stepped on a secondary and lost my lower left leg,? Todd said of the injury, which years ago might have immobilized him for good.

He is one of nearly 1,600 service members to lose limbs in combat since the start of the war in Afghanistan. Six months after his injury, Todd said he is ?almost back to new,? and has even started running and playing lacrosse again.

??I?ve got numerous prosthetics,? Todd said. ?I?ve probably got four or five different feet for different activities. I got one for ice skating. I got a running leg. You know, my everyday foot. I got a foot for hiking.?

The traumatic battlefield injuries sustained by troops on the frontlines have helped change the future for all amputees, doctors at Walter Reed said.

?We have plenty of examples from our injured service members who have not only survived, you know, extraordinary blast injuries but have thrived from them,? said Col. Paul Pasquina, chair of the center?s department of rehabilitation medicine. ?And there?s no reason to think that the victims in Boston won?t do the same.?

Whether it is bionic hands, knees, ankles, or feet, the advances at Walter Reed have been born of a decade of brutal conflict in which explosions have claimed lives and mangled limbs. While recovery often remains a painful process, the prospect for patients who have lost arms or legs is better than ever.

?While there have been significant advances in rehabilitation medicine and prosthetic technology over the last decade, that?s not to say recovery from a major limb loss is not extremely challenging, but there?s great hope,? Pasquina said. ?And people are now able to achieve things that they weren?t able to achieve in the past.?

Among those who have overcome seemingly insuperable odds is Travis Mills, one of five quadruple amputees from the wars in Iraq and Afghanistan. A service member in the 82nd Airborne Division, Mills was on a walking patrol on April 10, 2012, when an improvised explosive device went off as he stopped for a break.

?I sat in the wrong spot,? Mills said. ?And an IED went off.?

It was Mills? third tour in Afghanistan. He had a wife and baby daughter not even a year old at home. Now he can help the 18-month-old girl brush her teeth in the morning.

?My daughter, that?s my biggest support,? Mills said. ?The biggest thing I work for is to go every day to get better so I can be the best dad I can be for her.?

The cost of prosthetics can run from a few thousand dollars to an estimated tens of thousands and beyond. And while it?s unclear whether insurance will cover these types of prosthetics for the marathon victims, they have more options than ever.

?I?m very fortunate that the research that has been done has benefited myself due to my injuries,? Mills said. ?I know that I would?ve got hurt like I did 10 years ago ? I probably wouldn?t have made it off the battlefield.?

Whether the injured come from battlefields halfway around the world or a sidewalk on Boylston Street, traumatic wounds are often accompanied by deeper scars, said Dr. Harold Wain, chief of Walter Reed?s psychiatry consultation liaison service.

?They need to have a good perspective of who they are. They can feel good about themselves. They have to accept themselves,? Wain said.

?We?re constantly learning. There are new advances going on in prosthetics, in treatment, in medications,? Wain said. ?The goal is to get them back as whole, as quickly as possible, and to reinforce them for their assets rather than just looking at their liabilities.?

For Calvin Todd, he only needs to look to his side for inspiration. While the landscape of Afghanistan is a long way from Massachusetts, this war veteran knows what the Boston victims have to overcome and what they have to look forward to.

?There?s a lot you can do. The sky?s the limit,? Todd said. ?You can do anything you want to do, just work for it."

Federal government recommends removing dams from Klamath River in southern Oregon and Northern California: Hoopa Tribe says it will sue over proposed flows this year

GRANTS PASS, Ore. -- The federal government on Thursday recommended that all four aging hydroelectric dams be removed from the Klamath River in southern Oregon and Northern California to help struggling wild salmon runs, and that nearly $1 billion should be spent on environmental restoration.

However, whether that will happen remains in doubt. Legislation authorizing the secretary of Interior to approve dam removal and appropriating $800 million for restoration work have not gained any traction in Congress.

Meanwhile, conservation groups announced their intention to challenge the Bureau of Reclamation's plan to release water from the Klamath dams, which they say is insufficient to support coho salmon runs.

Karuk Tribe Klamath Coordinator Craig Tucker praised the report, saying that taking down dams was ?not only good for fish but safe, affordable, feasible and in the economic interest for power customers.?

The hydroelectric dams are owned by PacifiCorp and produce power for 70,000 customers.

As one of his last acts before leaving office, Interior Secretary Ken Salazar called on Congress to take action, saying that removing the dams and implementing the Klamath Basin Restoration Agreement are important components of finding a solution to the basin's water problems.

?Once again, the communities of the Klamath Basin are facing a potentially difficult water year under a status quo that everyone agrees is broken,? Salazar

said in a statement. ?We need a comprehensive solution addressing all the needs of the Klamath Basin, including fisheries, agriculture, (wildlife) refuges, and power.?

The battle to remove the dams is far from over. Newly elected conservatives on the Klamath County Board of Commissioners have voted to get out of the restoration agreement and House Republicans have not allowed the dam-removal measures to go to a vote.

But at least one lawmaker -- North Coast Congressman Jared Huffman, D-Santa Rafael -- has expressed support for dam removal.

?Every previous study from a wide range of sources informed this report and the scientific and common sense conclusion is clear: We should tear down these dams,? Huffman said in a statement.

?We are encouraged by the signals we are getting from Washington,? Josh Saxon of the Salmon River Restoration Council said in a release. ?During (newly appointed secretary of the Interior Sally Jewell's) confirmation hearings, (Oregon Sen. Ron Wyden's) first question focused on the Klamath. This tells us that both Congress and the administration are aware of the need to solve the Klamath crisis.?

Battles over how to share scarce water between farms and protected fish reached a head in 2001, when drought forced the U.S. Bureau of Reclamation to shut off water to a federal irrigation project straddling the Oregon-California border in an effort to provide water for endangered sucker fish in the project's main reservoir. Salmon in the Klamath River were also threatened. The next year, the Bush administration restored irrigation, but tens of thousands of adult salmon died when they returned to a river with low and warm water levels.

In 2010, tribes, farmers, salmon fishermen and conservation groups ended a century of fighting over water by signing the Klamath Basin Restoration Agreement, which called for the sharing of water in dry years and the removal of four dams to open up hundreds of miles of salmon habitat shut off for a century. PacifiCorp agreed to the removal rather than pay millions of dollars for fish ladders and other improvements.

?This final report confirms that dam removal is both feasible and cheaper than any other option,? Glen Spain of the Pacific Coast Federation of Fishermen's Associations, a salmon fishing group, said in a statement. ?None of the many scare stories spread by dam removal opponents were found to have any factual basis.?

Jeff Mitchell, a member of the Klamath Tribes tribal council, said their community was ?headed for a real train wreck? if action wasn't taken quickly.

The Hoopa Tribe, along with several other conservationists, has opposed the restoration agreement, saying they would prefer to seek dam removal by other means.

?The Hoopa Valley Tribe fully supports Klamath dam removal. We do not support dam removal being tied to water sharing legislation that lowers flows for salmon and sacrifices tribal water rights,? Hoopa Valley Tribe spokeswoman Regina Chichizola wrote in a statement. ?The Klamath Basin Restoration Agreement needs to be de-coupled from dam removal.?

In a opinion piece published in the Times-Standard last month, Hoopa Tribal Councilwoman Hayley Hutt said water rights granted in the agreement would trump tribal rights.

?Couple this with the fact that the KBRA allows flows that are much lower then current (Endangered Species Act) mandated flows, and that the KBRA lasts for 50 years, and it is apparent the agreement gives up too much,? Hutt wrote.

As the debate over the restoration agreement continues, the Hoopa Valley Tribe and conservation groups WaterWatch and Oregon Wild filed a 60-day notice of intent to sue the government, saying it had improperly imposed lower guaranteed flows of water for salmon on the Klamath River.

?The bureau has begun implementing a water management regime that cuts water to threatened coho salmon in the Klamath River, and to fish and wildlife elsewhere in the Klamath Basin, before the completion of a legally-required scientific and environmental review,? the groups said in a release.

This year is expected to be strong for returning salmon and commercial, sport and tribal fishing.

?The bureau is repeating the same mistakes that ultimately led to the 2002 Klamath River fish kill and the Klamath-driven salmon fishery disaster of 2006, and we are putting the agency on notice that a repeat of those tragedies is simply not acceptable,? Oregon Wild Conservation Director Steve Pedery said in the release.

LOUISVILLE, Ky. (AP) ? Kevin Ware is pretty certain how his next few months will play out.

"After we win the national championship," the injured Louisville guard said with a smile on his face, "I'm just looking forward to rehab."

Ware is already set for his next step, though he is gingerly walking around on crutches and with his surgically repaired broken leg in a cast up to his right knee: He is heading to the Final Four.

Ware was cleared Wednesday by doctors to accompany the Cardinals on their hour-long flight to Atlanta. The sophomore plans to be a full participant in preparations for Saturday's game against Wichita State.

He said the overwhelming support he has received has helped him maintain his spirits and strengthened his confidence of a full recovery. He hopes by next season to be helping the Cardinals defend the national championship he believes they'll win this weekend.

The normally reserved 20-year-old calmly recalled how he felt when he suffered the devastating injury, saying he doesn't think Louisville would be in the Final Four if he had lost his composure.

Ware credits teammate Luke Hancock for calming him down.

"He got me to that point where I really had to put the pain on hold," said Ware, with his leg in a cast propped up on a couch. "Once he said his prayer, I was kind of thinking the whole time, 'you can either be a crybaby about it or you're going to get your team back and get them in the right mindset'.

"Luke said his words, and I just kept repeating, 'y'all gotta go win this game.' I'm fine. ... It really helped the team."

But it wasn't easy for the Cardinals, many of whom cried after seeing Ware's gruesome injury ? his bone protruding six inches through his skin.

Even Louisville coach Rick Pitino was emotional, wiping tears from his eyes and later saying that the sight of his player's injury almost made him vomit.

But Pitino said everyone's emotions have settled down knowing that it appears Ware will be OK.

"I think we're all fine now," Pitino said. "Just having Kevin around, we can exhale."

The coach said having Ware in Atlanta might provide the Cardinals with a little "extra emotion," but in his experience "the team that executes the best will win."

Pitino and his son, Richard, spent Monday at the hospital with Ware, who was pictured holding the championship trophy in his bed. Though Ware had maintained his composure talking with AP, he became very emotional during an earlier interview with ESPN when talking about waking up and seeing the trophy.

The coach downplayed staying with him at the hospital after his injury.

"There's not a coach in America that wouldn't be there," he said.

And while Pitino said everyone can exhale now, the Cardinals had to take a deep breath when Ware went down on Sunday. They eventually regrouped and took the lead at halftime against Duke en route to an 85-63 victory over the Blue Devils in Indianapolis.

Through it all, Ware said he had to remain strong. He was placed on a stretcher and wheeled out of Lucas Oil Stadium to cheers of 'Kevin Ware, Kevin Ware,' before heading to Methodist Hospital.

Ware underwent a two-hour operation to repair compound fractures of the tibia that left the leg at an odd angle. He awoke the next morning to discover he had become an overnight sensation, and the afterglow hasn't waned.

His condition and progress have been featured every day on the major networks, the Internet and especially social media. The Cardinals' practice facility was surrounded by a phalanx of satellite trucks, and the interview requests helped Ware get an early jump on his rehab as he shuttled back and forth between makeshift sets.

"It's going to take a long time to get where I want to be," Ware said.

Not that he's dreading the hard work ahead.

"I think God puts things in your life and you have to go through certain obstacles," he said. "I just feel like these are obstacles that are going to make me grow up for the better. It's going to open my eyes to a lot of things I probably haven't seen before."

Ware said he has heard from several of his NBA idols, including Kobe Bryant, Kevin Durant and Charles Barkley. The Louisville guard said he has even heard from first lady Michelle Obama and the Rev. Jesse Jackson.

For the soft-spoken Ware, the support and media attention has meant more interviews than he ever imagined.

"I had no idea I would get this kind of attention," he said. "I'm one of those guys who just likes to play basketball. But the injury opened up a lot of peoples' eyes and I really appreciate all the support. It really means a lot."

But as Ware cherishes the flood of warm wishes, he's also dealing with the irony of the injury's occurrence with 6:33 remaining in the first half against Duke.

He leaped high near the right sideline to defend a 3-point attempt, similar to a defensive play he made without incident in Louisville's game in November against Duke in the Bahamas. This time he landed awkwardly, with the leg going in two different directions.

"That was frustrating because it happened the same exact way, me making the play," Ware said. "I was thinking then about just blocking the shot and that was what I was thinking this time. This was just different."

Ware also lamented the timing of his injury, a recollection that made him pause for a moment. A key part of Louisville's guard rotation who often substituted for starters Peyton Siva and Russ Smith, Ware had overcome a one-game suspension in January and was coming off a career-best, 11-point effort in Friday's tournament win over Oregon.

Pitino said that performance typified Ware's maturation process.

"Kevin has gone from being a quiet, unsure guy to being a very mature man," the coach said. "Kevin was very quiet, he kept to himself and didn't show many emotions. In the last couple of months, he's come out of his shell and is showing his emotions."

Ware is also finding out how difficult it is getting around with one healthy leg.

Fortunately for him, his girlfriend, Louisville sophomore Brittany Kelly, has been there to help since he was injured. Ware's teammate and roommate, forward Chane Behanan, will lend a hand as well.

"He's handling it better than I would've expected," Kelly said. "When they took the towel off his leg, he asked if he'd be able to play next week before they told him no."

Ware's mother, Lisa Junior, also plans to move from Georgia to aid her son's healing process. Ware said his leg will need eight to 12 weeks to heal before he begins rehabilitation in hopes of returning by the start of practice in October.

In the meantime, he's preparing for a homecoming in Atlanta, where he played high school ball after moving from New York. Ware foresees a working weekend with teammates intent on closing the deal in their second straight Final Four after losing last year's semifinal to rival Kentucky.

How he'll celebrate if the Cardinals fulfill their mission is unclear. Ware joked at climbing the ladder to cut down the nets but will likely settle for just being part of the action.

"Whatever I see on the court," he said, "I will tell them about it."

North Korean leader Kim Jong-un continued what has become a daily effort to raise tensions and fears abroad, and to consolidate patriotic ardor and unity at home, by saying he plans to restart an old nuclear reactor that can produce plutonium used in the creation of nuclear weapons.

US moves on NKorea aimed at deterring new leader

WASHINGTON (AP) ? The parading of U.S. air and naval power within view of the Korean peninsula ? first a few long-range bombers, then stealth fighters, then ships ? is as much about psychological war as real war. The U.S. wants to discourage North Korea's young leader from starting a fight that could escalate to renewed war with South Korea.

Worries in Washington rose Tuesday with North Korea's vow to increase production of nuclear weapons materials. Secretary of State John Kerry called the announced plan "unacceptable" and stressed that the U.S. is ready to defend itself and its allies. But he and other U.S. officials also sought to lower the rhetorical temperature by holding out the prospect of the North's reversing course and resuming nuclear negotiations.

At a joint news conference with visiting South Korean Foreign Minister Yun Byung-se, Kerry said the U.S. would proceed "thoughtfully and carefully" and in consultation with South Korea, Japan, China and others.

Defense Secretary Chuck Hagel, in a call late Tuesday to China's defense minister, called the North's development of nuclear weapons a "growing threat" to the U.S. and its allies.

Hagel, citing North Korea's pursuit of nuclear weapons and ballistic missiles in his phone conversation with Chang Wanquan, said Washington and Beijing should continue to cooperate on those problems, according to a Pentagon statement describing the call.

Michael Green, an Asia specialist at the Center for Strategic and International Studies, said it should be no surprise that North Korea is vowing to restart a long-dormant nuclear reactor and ramp up production of atomic weapons material.

"This is part of their protection racket," Green said in an interview. "I think the end state North Korea would like is that we, the U.S. in particular, but also China, Japan, South Korea, are so rattled by all this that we decide it's just better to cut a deal with them."

Tensions have flared many times in the six decades since a truce halted the 1950-53 Korean War, but the stakes are higher now that a defiant North Korea appears to have moved closer to building a nuclear bomb that could not only threaten the South and other U.S. allies in Asia but possibly, one day, even reach U.S. territory.

That explains, in part, why the U.S. is displaying military muscle to warn the North to hold its fire.

Washington also wants to leave no doubt that it has the South's back, and that Seoul should not act rashly. Nor does the U.S. want South Korea to feel compelled to answer the North's nuclear drive by building its own bomb.

"We are in the business of assuring our South Korean allies that we will help defend them in the face of threats," Pentagon press secretary George Little said, adding, "We are looking for the temperature to be taken down on the Korean peninsula."

Even without nuclear arms, the North poses enough artillery within range of Seoul to devastate large parts of the capital before U.S. and South Korea could fully respond. The U.S. has about 28,500 troops in the South, and it could call on an array of air, ground and naval forces to reinforce the peninsula from elsewhere in Asia and the Pacific.

In just the past few months, North Korea has taken a series of steps Washington deemed provocative, including an underground nuclear test in February. In December the North Koreans launched a rocket that put a satellite into space and demonstrated mastery of some of the technologies needed to produce a long-range nuclear missile. And several weeks ago, the North threatened to pre-emptively attack the U.S.

Bruce Bennett, a specialist in North Korean affairs for the RAND Corp., said he believes much of the recent taunting from North Korea reflects turmoil among the ruling elite in Pyongyang. He cited unusually high turnover among senior officials during the 15 months that Kim Jong Un ? grandson of the nation's founder ? has been the top leader.

"I think with the purges going on, he's got some instability that is generally not being recognized" outside of North Korea and that may be pushing Kim to take a more confrontational stance, Bennett said in an interview. "He's trying to be blustery to make it appear that he's really in control, he's really strong and he can defeat us."

In response, the Pentagon announced it would beef up missile defenses based on the U.S. West Coast, and it highlighted over a period of days the deployment of B-52 and B-2 bombers, as well as two F-22 stealth fighters, to South Korea as part of an annual U.S.-South Korean exercise called Foal Eagle, which lasts through April.

On Tuesday, officials said the Navy was keeping the USS Decatur, a destroyer armed with missile defense systems, in the vicinity of the Korean peninsula for an unspecified period instead of continuing its journey back to the U.S. after a Mideast deployment. And they said a similar ship, the USS McCain, had been shifted slightly to the waters off the southwest coast of the Korean peninsula as a further precautionary move.

North Korea has been an enigma to most outsiders since it was founded by Kim Il Sung in 1948. The United States has often misjudged the North's political path. After the founding Kim died in 1994, for example, U.S. intelligence officials said they believed his successor son, Kim Jong Il, would be more accommodating to the West.

"Flaky as he may be, (Kim Jong Il) nevertheless ... realizes the only way they're going to extricate themselves from the shambles that their economy is in now is to get outside help," James R. Clapper Jr. told a congressional panel in January 1995. Clapper was director of the Defense Intelligence Agency at the time; today he is President Barack Obama's most senior intelligence adviser as director of national intelligence.
